Insurance Policy
Due to insurance company limitations we do not accept insurance assignment as payment. We will help you file your claim so that your insurance company may reimburse you.
Cancellations
Please give at least 24 hours notice for appointment changes or cancellations. Otherwise, charges may be applied.
Payments
It is the policy of this office that fees for treatment are due in full at time of service. We are pleased to accept cash, check, VISA and MasterCard for your convenience. If you have any questions regarding payment please do not hesitate to contact our office.
